Important reminder on home modifications assessments
Please remember these important points before you conduct a home modifications assessment for a TAC client.
When we consider home modifications for a TAC client, an occupational therapist uses their experience in home modifications to assess the home and look at the client’s needs and goals.
Our dedicated TAC home modifications team is available to support you and the client at every step in this process.
Please remember these important points before you conduct a home modifications assessment for a TAC client:
- You must communicate with and receive approval from a TAC home modifications representative before you complete a home modifications assessment.
- Your assessment must be conducted with input from the TAC home modifications representative, project manager and/or builder at the site visit.
We may not be able to pay for assessments that do not meet these conditions.
The process helps us ensure that the home modifications are:
- aligned to the client's entitlement based on their transport accident injuries,
- in accordance with our policy and what we can reasonably fund and
- feasible
Once the approval for the assessment is in place, the home modification representative will direct you to our updated Home modifications assessment and recommendations form.
