Measure client health outcomes with expanded resource

Treatment plans
Allied health providers may be asked to complete the new Allied health treatment and recovery plan and psychologists may receive requests to complete the Mental health treatment plan.
These plans help us better understand our client’s treatment needs and goals in line with the TAC Clinical Framework. Treating providers should set outcome measures for all TAC clients and regularly review their progress.
Outcome measures
A new suite of outcome measures is now available on our website to help you assess a TAC client’s current or future health status and demonstrate the effectiveness of treatment. You can download and print each measure, or complete and score it online. All are free to use.
Members of our Clinical Panel recommend these measures as being particularly useful in clinical practice. This body of highly experienced allied health, disability, mental health and medical professionals can provide you with support and advice as you treat TAC clients.
Gap in treatment
If a client hasn’t accessed any TAC funded treatment or services in 6 months, they’ll need to contact us before we can consider paying for further treatment.
This will allow us to discuss with the client how they’re progressing toward recovery. Please ask the client to contact us if 6 months have passed since they last accessed treatment.
